Is this pleurisy? Pneumonia? Any advice welcome.?

I had a cold the 3rd week in January. The week after that, I came down with acute bronchitis. I went to a doc-in-a-box, who gave me a cough suppressant & zithromycin. I was still having problems breathing, went to my regular doc a few days later. He gave me an inhaler, which helped at first, but not so much now. I'm now having shortness of breath after walking 20 steps or so, and shoulder blade pain when I inhale. My fever is gone, so I'm guessing it's not pneumonia. Do I call my doc tomorrow morning & report the new symptoms & try to go in to see him? Do I tough it out? I've been off work for a week - do I go back tomorrow, work a few days & see how it goes?
